Shift: 8:00 AM - 4:30 PM

Experience:

• 36 months of Case Management experience is required.

• 3 months of experience with Meditech is required.

Requirements:

• Candidates must have a Illinois license in hand at the time of application.

• This role will require floating to multiple units

• Flu vaccination required for onboarding. Religious, medical, and personal declinations accepted.

Additional Details:

Bi-weekly orientation

Guaranteed Hours: biweekly guarantee- 72/80 hours

Float Requirements: will work throughout all units of the hospital

On Call/Call Back Requirements: No

Weekend/Holiday Requirements: Weekend coverage every 3rd-4th weekend & Holidays as needed

Required Education: BSN

Minimum Experience: 3-5 years of current hospital acute medical Case Management/Utilization Review experience with discharge planning

Charting Experience Required: Meditech Expanse – RNs are required to complete Step 1 of modules prior to start date with facility. Steps 2 & 3 completed at facility.

Required Skillset: UR experience with Interqual and EQH (MUST must have Hospital Utilization Review experience with InterQual. Will not teach individual how to use InterQual) .Through review of the medical record determines patient’s severity of illness, medical necessity and need for acute care. Advocates for adherence to appropriate evidence based practice standards by querying/prompting physicians and nursing to implement/order/document appropriately to support the severity of patient illness. Monitors length of stay, resource utilization, quality of care, timeliness of service through comprehensive manual chart review.

Unit Information (Bed size, Case type, Patient ratio): Case Type-Cases are all medically based. Case load-Varies by census and staffing, at full staffing 15-18 pts, social worker can go up to 30, also includes ED and L&D/ICN
