Physical Requirements:

A high degree of digital and manual dexterity required in most aspects of the job. Also, constant standing, walking, frequent bending and stretching, occasional lifting of items weighing up to 75 pounds and occasional pushing of items weighing up to 200 pounds. Exposure to bodily fluids, communicable diseases, toxic substances, medicinal preparations and other conditions common to a clinic environment that includes laboratory.

Qualifications:

The Licensed Practical Nurse (LPN) must hold an active license as a LPN in the state of West Virginia. LPN should have excellent verbal and written communication skills and effective interpersonal communication skills. LPN should have basic computer skills.

Basic Function:

Responsible for assisting providers with direct patient care through clinical duties and telephone care. Assist medical support staff with clerical duties, computer data entry, referrals, procedures within scope of practice, and routine or complex patient communication.

Duties and Responsibilities:

  • Demonstrate competency with required trainings through completion of three orientation checklists: MA/LPN job description checklist, phone triage checklist, and electronic health record (EHR) documentation.
  • Perform routine and emergent/urgent telephone and walk in triage and forward information appropriately to Provider/MA/Nurse.
  • Perform patient assessments, including vital signs, blood pressure, height, weight, BMI, pulse oximetry, pulse, chief complaint, and age appropriate screenings.
  • Review, update, and reconcile allergies, medication list, problem list, reminders, family history, and surgical history.
  • Perform/complete quality management as applicable per patient, including self management, goal setting, and follow up. Provide educational material for patients with chronic illness, or in need of prevention education materials.
  • Clean and stock nurse's station, lab, patient bathrooms, equipment, exam rooms daily, as well as other areas of the site as needed.
  • Exam rooms are to be cleaned and disinfected between every patient daily.
  • Attend and participate in staff meetings and requested committees.
  • Perform additional duties as assigned.

Provider Directed Responsibilities:

  • Perform all on-site testing as ordered (or per standing orders) and document results into the (EHR) and bill, including UA, UPT, strep screen, hemoglobin, blood glucose, Hgb AIC, rapid mono, flu, hemoccult, and drug screens.
  • Perform venipuncture label tubes appropriately and route all specimens to correct lab according to payer source if needed.
  • Perform EKG, oxygen administration, nebulizer administration, pulse oximetry, cryo procedure assistance, and tympanogram.
  • Administer adult and childhood vaccines under the direction of the in-house provider and/or per the approved standing orders of the health center.
  • Administer medications under the direction of the in-house provider and/or per the approved standing orders of the health center.
  • Perform ordered screenings, including hearing, vision, and TB testing.
  • Obtain prior-authorization for prescriptions, durable medical equipment, and other therapies as needed, and forward appropriate documents.
  • Assist providers with patient communication including lab and test result notifications, prescription call-ins, and patient call backs.
  • Administer wound care and dressing changes.
  • Insert and maintain urinary catheters.
  • Insert and discontinue IV catheters (certified).
  • Ensure patient safety by monitoring and assessing the patient in urgent situations.
