Shift: 4x10s or 5x8s, M-F, 7am-5pm (will be 40 hours/week)
Day off per week cannot be Monday or Friday
85% in clinic/15% at home visits
The Community Care RN provides and supervises professional, independent nursing care for geriatric patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. This program is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Home health experience
2+ years as a RN, ideally working with mostly geriatric patients
ASN or BSN
RN license in the state of Oregon
BLS certification (upon hire)
Personal vehicle and ability to travel in the community to see patients (mileage is reimbursed)
