VNA Hospice of Philadelphia is seeking an experienced hospice nurse to provide clinical education and to actively participate in the agency’s quality assurance/process improvement activities. In this role, you will work close with the hospice interdisciplinary team to ensure high quality standards for clinical care and documentation are achieved. The ideal candidate must be highly competent in care delivery for end-of-life patients with superior documentation and teaching skills as well as electronic medical record (EMR) skills.

Position Summary: Responsible for assigned quality assurance and performance improvement activities within the hospice department and provides education and training for hospice clinicians and support staff.

Essential Functions:

Provides guidance for clinicians to achieve accurate assessments and documentation and plan of care development.
Provides clinician specific education regarding quality, outcomes improvement, documentation standards, and actions to improve clinical processes.
Participates in department quality improvement initiatives to achieve the Agency’s strategic goals and top clinical outcomes.

Knowledge and Experience:

Extensive knowledge of Medicare conditions of participation and accreditation standards.
Comprehensive knowledge of all federal and state regulations.
Sufficient knowledge of professional nursing standards, concepts, policies, and practices for hospice care.
Ability to recognize learning needs of clinical and support staff and develop educational content to address improve staff knowledge base.
Understands and demonstrates the principles of adult learning.
Demonstrates super user level competence with the Homecare Homebase documentation system.
Prior hospice or healthcare quality experience is a plus.
Current PA RN license.
Minimum of 3 years hospice clinical experience.

Education: RN diploma, AD in nursing or BSN.
