Job Description
Responsibilities

The Manager, Social Work is a member of the Care Management team and collaborates with all members of the patients care team to support the delivery comprehensive care services. As an independent professional, the manager will also assess patients' psychosocial and continuum of care needs and provide direct interventions to help address identified needs. The manager collaborates with community partners to help patients/families gain access to services that to help address medical and social care needs. The manager adheres to all Norton Healthcare policies and the NASW code of ethics in the delivery of social work services. The manager supports the professional development of department staff and help ensure the delivery high quality equitable care. In collaboration with the Director, Clinical Effectiveness and Care management is responsible for care management services while ensuring patients receive cost-effective, high quality, and safe care. Care management services include, but are not limited to, care coordination, discharge planning, and psycho-social services. The manger also works in collaboration with key stakeholders to drive initiatives that support quality services across the care continuum.

Key Accountabilities:

  • Delivers patient-centered care by developing a plan of care in collaboration with patient, family and healthcare team. Provides and advocates for trauma-informed services. Serves as a champion for the provision of fair and culturally informed services. Empowers patients/families as key stakeholders in the self-management of illness, treatment, and services.
  • Provides skilled interventions including, but are not limited to; counseling, navigation of family dynamics, crisis intervention, de-escalation, reporting/navigating matters of suspected abuse/neglect, guardianship issues, substance use services, coordination of mental health services, addressing intimate partner violence, grief/loss support, legal concerns, social/environmental circumstances, financial concerns, transportation needs, and other social determinants/drivers of health as needed. Provides liaison services and collaborates with members of the patient care team including, but not limited to, physicians, nurses, behavioral health professionals, community health workers, and other service providers. Advocates and supports patients/families through life-changing health events. Provides ongoing support to patients/families through the treatment process. Coordinates with external partners to support medical and social care across the care continuum.
  • Conducts comprehensive psychosocial assessments with designated patients/families. Assesses concerns for possible abuse/neglect. Uses critical thinking and systems perspectives to assess and navigate care.
  • Responsible for daily operations. Mentors staff through the coordination of orientation, ongoing coaching/education, and performance evaluation. Oversees staffing productivity, schedules to ensure the effective utilization of facility care management staff. Facilities the achievement of the annual care management budget and performance metrics developed collaboratively with leadership. Works in collaboration with the department director to achieve Norton, as well as departmental, goals and objectives. Audits records to help insure documentation meets professional standards.
  • Leads formal groups in rapid-cycle and performance improvement activities.

Qualifications

Required:

  • Three (3) years of medical social work experience
  • Master Degree in Social Work
  • Certified Social Worker (CSW)
