Position Details:

Full-Time, hours of typical operation are M-F, 8:00am-5:00pm. Must be able to work a flexible schedule. Ability to volunteer for the on-call crisis response team as needed.

Position Summary:

A Qualified Mental Health Associate (QMHA) is a person who delivers services under the direct supervision of a QMHP (Qualified Mental Health Professional). The Wraparound Care Coordinator is responsible for developing, coordinating, leading, and facilitating child and family teams to support the highest risk children and families in the community. Services and supports are intended to assist youth to remain in or return to their homes and schools in the community. This position works with a team who values the development of family-driven, child-focused continuum of services when working with youth and their families. The Wraparound Care Coordinator also assists families in developing comprehensive crisis/safety plans and accessing resources and services as needed to divert placement or hospitalization of youth.

Requirements

Education:

Bachelor's degree in a behavioral sciences field; or a combination of at least three years relevant work, education training or experience; and able to demonstrate the competency necessary to fulfill the position. Prior experience with System of Care and Wraparound program preferred.

Licensure/certification:

Must be eligible for certification as a Qualified Mental Health Associate (QMHA) through the Mental Health and Addiction Certification Board of Oregon (MHACBO). Valid Oregon Driver's license with a clean driving record for the past three years. Must pass all criminal history check requirements as required by ORS 443.004. Have or obtain CPR/BLS certification. Must obtain GOBHI certification in Wraparound Foundational Training. Must obtain certification in the CANS assessment process.

Experience:

Mental Health case management experience for 2 years is preferred. Direct involvement in treatment and knowledge of working with mentally ill clients, or any satisfactory combination of experience in working with mentally ill adults and/or youth and families. Experience in providing wraparound services preferred. Cerner or other eHR systems experience required. Microsoft Office Suite experience required.

Job Knowledge/Skills:

Ability to identify, advocate and coordinate varying levels of care, treatment and transitional care for families enrolled in the Wraparound Program. Understand and apply skills necessary to place individuals in residential treatment if needed and be able to identify resources and solutions to maintain children in their homes and communities. Exhibit the ability to learn and apply new skills. Use resources effectively, maintain professional boundaries and demonstrate the ability to adapt to situations appropriately. Possess problem solving and solution focused skills.

Must have skills in interacting with the public and the ability to coordinate services across various agencies and community groups. Must have the ability to communicate effectively, verbally and in writing. Flexibility and ability to work effectively with youth, families, adults, and in collaboration with community agencies. Must be computer literate. Must understand HIPAA regulations. Be able to upload information to a GOBHI spreadsheet and report on Wraparound Fidelity Index (WFI-EZ)
