Hospice RN Case Manager

Russell-Murray Hospice is seeking a compassionate RN to join our team as an RN Case Manager for our Kingfisher Office! The RN Case Manager provides exceptional professional palliative and supportive nursing care, while playing an essential role in the Interdisciplinary Team. This position supervises and directs the care as well as coordinates the overall hospice plan of care for each patient. We are a free-standing non-profit hospice who puts patient needs first. • Full-time position
• Monday - Friday 8:30a - 4:30p.
• Position is for our Kingfisher Office
• Full benefits package offered
• Paid Holidays
• Mileage reimbursement.
• Serve a 75-mile radius of El Reno Office. (4 locations - El Reno, Kingfisher, Weatherford, Oklahoma City)
• You may be asked to float to other offices at times. Flexibility is a must.
• Must have a reliable car and personal auto insurance and a valid driver's license.

Responsibilities:

• Create and implement hospice plan of care for each patient.
• Ability to provide skilled nursing assessment and intervention to terminally patients.
• Provide continuous care for symptoms management or imminent death.
• Provide emotional/therapeutic support to families during difficult times.
• Delegate appropriate tasks as needed
• Educating family and caregiver on plan of care, medication regimen, wound care, etc...
• Provide education on end-of-life care and disease progression.
• Playing a vital role in Interdisciplinary Team Meetings, creating goals and assisting patients with meeting them.
• Accurately and timely complete all documentation according to RMH policies and procedures.

Requirements:

• Valid Oklahoma Nursing License (RN)
• Reliable vehicle with personal auto insurance and a valid driver's license.
• Live within service area (75-mile radius of El Reno)
• Experience in nursing and dealing with death and dying patients.

Nice to have:

• Experience documenting in electronic medical records systems.
