Registered Nurse Home Care

We are seeking a compassionate and skilled Registered Nurse to join our home care agency. The ideal candidate will be responsible for providing high-quality medical care to patients in the comfort of their own homes. As a home care nurse, you will assess patients' health needs, develop care plans, administer medications, and educate patients and their families on proper health care practices. This role requires excellent communication skills, attention to detail, and the ability to work independently.

Responsibilities

• Conduct initial assessments of patient's health status and develop individualized care plans.

• Perform regular follow-up visits to monitor patients' progress and adjust care plans as needed. Requires traveling. 

• Administer medications, treatments, and therapies prescribed by physicians.

• Provide skilled nursing care, including wound care, catheter care, and medication management.

• Educate patients and their families on disease management, medication adherence, and preventive healthcare measures.

• Collaborate with other healthcare professionals, such as physicians, therapists, and social workers, to coordinate patient care.

• Maintain accurate and up-to-date medical records and documentation.

• Ensure compliance with all relevant regulations and policies governing home healthcare services.

• Provide emotional support and encouragement to patients and their families.

• Participate in ongoing training and professional development activities to enhance nursing skills and knowledge.

Requirement

• Valid Registered Nurse (RN) license in the state of South Carolina.

• Excellent assessment and critical thinking skills.

• Strong communication and interpersonal skills.

• Ability to work independently and prioritize tasks effectively.

• Reliable transportation and a valid driver's license.

• Compassionate and patient-centered approach to care.

• Willingness to undergo background checks and drug screenings.
