Registered Nurse: Home Health Contract

Key Responsibilities:

• Collaborate closely with healthcare professionals to develop comprehensive home care treatment plans.
• Diligently record and monitor patient conditions and progress.
• Administer medications and provide essential wound care as necessary.
• Ensure accurate documentation using relevant systems, including HomeCare HomeBase (HCHB) and OASIS Platforms.

Requirements:

• Active RN licensure in Louisiana.
• Minimum of one year of recent experience in home health.
• Previous experience managing a home-to-home workload is advantageous.
• Familiarity with HomeCare HomeBase (HCHB) and OASIS Platforms preferred.
• Current BLS Certification from the American Heart Association.

Details:

• Engage in fulfilling home-to-home responsibilities within the Ville Platte, LA area.
• Full-time role, 40 hours per week, with shifts scheduled from 8:00 AM to 5:00 PM.
• Mileage reimbursement provided for travel within Ville Platte.
