Experience:

• 24 months of Case Management experience is required.

• Experience with Allscripts/Eclipsys is required.

Requirements:

• Candidates must have a New Hampshire license or compact license in hand at the time of application.

• COVID vaccination required for onboarding. Declinations not accepted.

Guaranteed hours: 40

Weekend Requirements: Every other weekend/ Holidays as needed

Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice experience.

RN will be working in the Whitefield/Berlin, NH area.

RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly).

Netsmart/Allscripts EMR experience strongly preferred.

Driver’s license required
