THE POSITION

We are seeking an experienced Aging Care Manager 2 to join our team of dedicated professionals. As an Aging Care Manager 2, you will play a vital role in providing high-quality care and support to elderly individuals within the community. DESCRIPTION OF WORK

In this position, you will be completing comprehensive functional ability assessments in order to determine the appropriate level of care for individuals wanting to apply for nursing facility care, personal and domiciliary care placement, as well as home and community-based services. This requires the ability to utilize proven interview techniques in order to gain accurate information from individuals and their representatives. You will be required to manage a large number of assignments each month, in regard to scheduling and completing assessments within the mandated time frame, and the necessary follow-up.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • This position may require a standby rotation schedule for off-hour coverage.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Allegheny, Armstrong, Cambria, Fayette, Indiana, Somerset, Washington, or Westmoreland counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
