Position Summary:

  • Performs patient interviews and assessments to determine psychosocial and discharge planning needs documenting care and services provided according to hospital policy.
  • Maintains current knowledge of resources available within the community maintaining supply of resource materials to be distributed to the patient and/or patient representative when needed.
  • Serves as a participant of the interdisciplinary team, forming and updating the plan of care and communicating the psychosocial and discharge needs of the patient.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health errors addressing opportunities for performance improvement.
  • Assists with discharge planning and processes, identifying appropriate resources including transportation, housing, healthcare and social/spiritual services provides referrals as part of the discharge plan.
  • Communicates plan of care with interdisciplinary team and patient/patient representative.
  • Provides support with advance directives and assists patient/patient representative with the grieving process making internal and community based referrals as needed.
  • Works as an advocate for the physical and emotional well-being of the patient protecting the patient's rights.

Reports to: RN-Nurse Manager Behavioral Health


Minimum education:

Masters Degree in social work required.

Minimum Work Experience:

Three or more years of experience in all aspects of social service and discharge planning preferred.

Required Skills/Certifications:

Current American Heart Association BLS certification or obtain within 90 days after hire date.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

Must be able to work in a stressful environment and take appropriate action.

Language Skills: Able to communicate effectively in English, both verbally and in writing
