Opportunity Specifics:

• Nurse Practitioner or Physician Assistant
• Location: Central Texas (Overseeing 14 Locations)
• Setting: Nursing Homes & LTC Facilities
• Schedule: Monday to Friday
• Patient Population: 100% Geriatric Patients

Responsibilities:

• Patient Assessment and Care Planning: Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams.
• Clinical Duties: Perform physical examinations, manage chronic conditions, prescribe medications, and provide wound care.
• Geriatric-Specific Care: Address unique needs of elderly patients, implement fall prevention strategies, and discuss end-of-life care.
• Collaboration and Communication: Work closely with healthcare professionals, communicate effectively, and participate in care conferences.
• Quality Improvement and Compliance: Ensure regulatory compliance, participate in quality initiatives, and monitor infection control practices.
• This dynamic role requires adaptability, compassion, and a commitment to improving the lives of elderly patients in Central Texas.
