Job Summary

Supports clinical and administrative functions of the integrated care team. Facilitates communication with and provides for available resources between the patients and the care team to address patient concerns, unmet needs, and engagement opportunities. Supports patients and providers within the community setting by performing basic direct patient care activities.

Job Duties

  • Implements the planned strategy designed to build relationships for new patients and retention of existing patients.
  • Provides guidance and assists patients during annual enrollment and redetermination periods.
  • Provides written and verbal responses utilizing the EMR to accurately reflect patients' needs/concerns and the care team's responses.
  • Collaborates and coordinates with medical staff, administration, clinical operations, and support staff to facilitate prompt resolution of patient-related concerns and service issues.
  • Provides and documents timely feedback regarding patient satisfaction,
  • Works directly with Administrative Team to develop annual outreach, enrollment, and engagement plan and all other appropriate materials.
  • Identify patients, in collaboration with care team, who are not fully engaged in care model to understand barriers to care and connect them with resources to address barriers
  • Provides basic first aid and blood pressure screenings as directed by the provider and clinical supervisor.
  • Attends and participates in staff meetings, in-services, projects, and inter-program committees as assigned.
  • Provides back up support to front office and non-licensed clinical team.

Hours: Days / Monday - Friday

No Evenings, Weekends, or Major Holidays

Education

High School Diploma or Equivalent (GED)- (Required)

Experience

Minimum of 5 years-Related work experience (Required)

Certification(s) and License(s)

Valid Driver's License - Default Issuing Body; Basic Life Support Certification within 6 months - Default Issuing Body
