This position will work with Homeless Outreach and Engagement Team to provide services to those experiencing homelessness. Service provision is community based and provided to individuals living in sheltered or unsheltered environments.

  • Serve adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder.
  • Field-based services with potential transportation of clients using agency or personal vehicles as determined by needs of consumers and/or program.
  • Meet clients where they are in terms of current mental health symptoms and treatment preferences
  • Provide medication reconciliation
  • Coordinate care and communicate freely with consumers' primary care providers
  • Provide nursing assessment and wellness services for consumers as appropriate.
  • Provide follow up and coordination from hospital or ER visits.
  • Work collaboratively with community partners
  • Provide services in-person or via telehealth platforms
  • Gather and complete all CCBHC data metrics as determined
  • To also include all duties and responsibilities outlined in primary RN/Nurse Care Manager job description.
  • Provide outreach and engagement to individuals experiencing homelessness in the community
  • Provide resources and support to connect individuals to essential services
  • Provide guidance, support, and advocacy in navigating complex systems
  • Collaborate with other outreach teams, local businesses, and other organizations to foster community engagement

    This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Education
Associates of Nursing (required)

Experience
1 - 2 years: Applicants will have previous background working in community mental health, inpatient psychiatric facilities, home health, or similar settings. Strong candidates with a passion for helping individuals with severe mental illness will also be considered. (preferred)
