Job Title:        Patient Outreach Coordinator             FLSA Status: Hourly, Non-Exempt

SUMMARY:

The Patient Outreach Coordinator ensures the overall success of physician’s treatment by collaboratively working with patients, physicians, and practice teams to provide the best care possible as described in the treatment plan. The majority of the job will consist of clinical data management using the Electronic Medical Record and outreach to patients and healthcare facilities to coordinate care as dictated by quality management reports, managed care reports, and outstanding clinical orders.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

·         Reviews and assesses patient data from Electronic Medical Records (EMR), chart reviews, or other information, to assist in the monitoring and facilitation of adherence to prescribed care plans

·         Assists the care team in data tracking, patient outreach, and data entry of patient communications

·         Collaborates with the patient as well as providers regarding opportunities for optimizing care

·         Facilitates patient understanding of the physician’s treatment plan, including but not limited to,

referrals, lab and diagnostic orders, and follow up care

·         Coordinate patient care and services to meet quality metrics

·         Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all

times, and follows all HIPAA guidelines and regulations

·         Participates in Quality Improvement (QI), Quality Assurance (QA) and Continuous Quality Improvement Activities (CQI) as appropriate

·         Other duties as assigned by management

QUALIFICATIONS/SKILLS:

High school diploma or GED

 

  • Able to effectively communicate with staff, physicians and patients.
  • Must provide excellent customer service.
  • Strong organizational and interpersonal skills
  • Must possess basic computer knowledge and skills

 

PHYSICAL REQUIREMENTS:

Hearing: Adequate to perform job duties in person and over the telephone.

Speaking: Must be able to communicate clearly to patients over the telephone.

Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

Other: Frequent sitting in a confined position; occasional lifting, bending, stooping, walking or standing with some heavy lifting for short periods of time. It is necessary to view and type on computer screens for long periods. Manual dexterity must be sufficient to operate a keyboard, type 25wpm, operate a computer, fax machine, calculator and other necessary office equipment.

 

WORK ENVIRONMENT:

Work is performed inside a temperature-controlled office building in an open cubicle office environment.  Work involves frequent telephone contact with patients.
