NURSE PRACTITIONER

General Summary:
The Nurse Practitioner will educate and advise patients, diagnose and treat health problems, and provide general health care under the direction of the physician or medical director. This position requires travel to patient’s homes and availability to float to different provider offices as needed. Focus is on Transitional Care Management and Medicare Wellness visits as well as follow up on high-risk patients.

Principal Duties & Responsibilities:
1. Complete TCM visits, MWV, and follow up visits in home setting for patients unable to get to provider office
2. Complete Medicare Wellness visits for Toledo Clinic providers
3. Assist with covering The Toledo Clinic primary care physician offices on as needed basis
4. Obtain medical histories.
5. Perform comprehensive medication reconciliation.
6. Identification of risk factors that contribute to rehospitalizations.
7. Diagnose and treat health care and chronic diseases.
8. Order and interpret diagnostic studies such as labs and x-rays.
9. Prescribes medications and other treatments.
10. Promotes positive health behaviors and self-care skills through education and counseling.
11. Collaborate with physicians as needed.
12. Emphasizes wellness and self-care to patients and family members.
13. Researches medical findings and studies and incorporates as appropriate.
14. Selects cost-effective and appropriate diagnostic fees.
15. Other duties as assigned.

Performance/Experience Requirements:
- Knowledge of nurse practitioner theory, practice and medical care, medical equipment and instruments, and OSHA regulations.
- Excellent interpersonal and communication skills.
- Ability to analyze and determine solutions in order to best serve the patients.
- Demonstrates adaptability to expanded roles.
- Consistently arrives at work, in professional attire, on time and completes all tasks within established time frame.
- Adheres to Toledo Clinic policies and procedures.
- Valid drivers license and current insurance.

Education:
- Masters degree from an accredited school.
- Certified as a nurse practitioner and CPR required.
