We are looking for a compassionate Licensed Practical Nurse to join our growing team! The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to our clients in their homes. We have open positions and are looking for the very best to support clients in their homes in the Metro Detroit Area, Macomb County and Oakland County.


The Ideal Candidate provides patient care planning, direct treatment and consultation to a general homecare caseload under the supervision of a Registered Nurse. Participates in the clinical interdisciplinary team process. Provides teaching and training to caregivers and staff. Involvement in clinical program development. Nurse provides nursing services to clients in the home and accepts accountability for his/her nursing practice.


Minimum Qualifications:

  • Strong organizational and analytical skills; oral and written communication skills.
  • Graduate of an accredited school of Nursing.
  • Current license from the State of Michigan to practice as a Licensed Practical Nurse and license is in good standing.
  • At least 1 year professional work experience as a LPN in a skilled/clinical setting.
  • Licensed driver with insured automobile which is in good working condition.
  • Possesses current CPR certification.
  • Current negative TB testing/screening
  • Freedom from illegal use of drugs; freedom from use of and effects of use of drugs and alcohol in the workplace.
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in the position.

Essential Functions:

  • Provides Nursing Services under the supervision of a Registered Nurse.
  • Able to provide the following skilled services in the home and able to instruct/teach patient and caregivers where necessary: Wound vac management, blood draws, wound care dressing changes, enteral nutrition setup and administration, medication administration, tracheostomy care, care planning and goal setting.
  • Care for multiple patients throughout the day in their individual home environments
  • Follow care plan and administer type and frequency of therapy treatments prescribed;
  • Report significant findings to RN Case Manager, Physician, and Clinical Manager
  • Follow Physician and RN orders in a timely, complete and accurate manner
  • Ensure the safety of the treatment procedures, patient and environment at all times
  • Demonstrates knowledge of medications and their correct administration based on the patient
  • Teach the patient and/or family members about new diagnosis, care, and support skills
  • Submit documentation on WellSky EMR in a timely, complete, and accurate manner
  • Attend in-services and meetings as required
  • Prepares clinical and progress notes, documenting client’s progress and response to nursing intervention and teaching while planning for client’s discharge from service.
  • Maintains communication with other disciplines to coordinate treatment goals and to support objectives outlined in the Plan of Care. Informs physician and other personnel of changes in client’s condition and needs, as warranted.
  • Reports to work on time and as scheduled
  • Maintain HIPAA compliance and patient confidentiality at all times
  • Position requires 100% travel to our patient’s home
