Person Center Planner

The Charles Lea Center in Spartanburg, SC is hiring a full-time Person-Centered Planner.

The Person-Centered Planner earns a competitive wage of $17.00/HR

The Person-Centered Planner functions under the general supervision of the Director of Individual and Family Support and Person-Centered Planning. The Person Center Planner will use assessments completed by the programs to determine the needs, wants, goals, and personal priorities to support each person we serve.

A DAY IN THE LIFE OF A PERSON-CENTERED PLANNER:

As a Person-Centered Planner, you will be responsible for the development and implementation of a comprehensive, collaborative person-centered plan that is reflective of the goals, dreams, and interests of the person supported. Will demonstrate respect for people with disabilities, their families, and advocate and maintain a positive interaction with other service entities within and outside the Center. Provide services and support focused on achieving goals for people support and monitor training goals and objectives for necessary revisions or changes as needed. Will facilitate meeting on behalf of the person supported to address any new needs or possible concerns with the services that they receive.

Requirements:

  • Valid SC driver's license
  • Be at least 18 years of age
  • Ability to pass a background check upon hire
  • Proficient in Microsoft office
  • Bachelor's degree with 3-5 years of experience developing person-centered plans
  • Frequent Travel required in the Upstate Area
