Summary: The primary responsibilities of your role in your assigned location is to provide direct nursing care to residents and to supervise the day-to-day nursing activities following the current federal, state and local standards, guidelines and regulations that govern long term care and skilled nursing facilities. You are entrusted to ensure that the highest degree of quality care will always be provided to our Villagers.

ESSENTIAL FUNCTIONS:

  • Coordinate resident care with other departments
  • Communicate changes in resident's condition with employees, physician, resident and family members
  • Perform administrative duties such as: completing medical documents, reports, evaluations, studies, charting, etc
  • Admit, transfer and discharge residents as required
  • Complete accident/incident reports

OTHER DUTIES:

  • Evaluate adequate supplies are available including but not limited to pharmaceuticals, resident personal supplies and equipment
  • Evaluate that resident abuse does not occur
  • Complete rounds to evaluate and assist in development and implementation various types of care needs on each resident including but not limited to; wound care, behavior management, respiratory care, etc
  • Provide treatments and care to medically complex residents in collaboration with the physician
  • Attend care plan meetings as assigned
  • Provide care plan updates as necessary to assure appropriate resident care
  • Provide resident care according to physician orders and care plan
  • Administer medications and treatments according to physician orders and standard of practice
  • Respond to all resident call lights regardless of assignment
  • Coordinate with outside agencies including but not limited to; hospitals, hospice, home health, lab, x-ray etc
  • Perform other duties as requested

EXPERIENCE:

  • One-year experience in a long-term care environment (preferred)

EDUCATION:

  • Current, unencumbered, active license to practice as an LPN in Colorado, CPR Certification

WORK ENVIRONMENT:

  • Move intermittently during working hours
  • Subject to frequent interruptions
  • Involved with residents, personnel, visitors, government agencies etc. under all conditions and circumstances
  • Subject to hostile and emotionally upset residents, family members etc.
  • Work beyond normal duty hours on weekends and in other positions temporarily when necessary
  • Subject to call back during emergency conditions (e.g. severe weather, evacuation, post-disaster etc.)
  • Subject to injury from falls, burns, odors etc., throughout the workday as well as reactions from dust, disinfectants and other air contaminants
  • Subject to exposure to infectious waste, diseases, conditions etc., including but not limited to TB, AIDS and Hepatitis B viruses
  • May be subject to handling of and exposure to hazardous chemicals

PHYSICAL AND SENSORY REQUIREMENTS:

  • Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times, hostile people within the facility
  • Meet the general health requirements set forth by the policies of the facility which include a mental and physical examination
  • Possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
  • Ability to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum of 50 feet
  • Ability to assist in the evacuation of Villagers during emergency situations

OTHER:

  • Ability to pass a criminal background check as well as Colorado CAPS background check.
  • COVID Vaccine Required or Valid Exemption.
  • Current, unencumbered, active license to practice as an LPN in Colorado, CPR Certification
