Job Description

PCHC is looking for a Medicare Annual Wellness Visit Specialist to serve patients at Helen Hunt Health Center in Old Town! As a MAWV Specialist, you’ll function as a direct-care clinician and collaborate within integrated care teams to identify and outreach patients who are eligible for wellness visits and/or have fallen behind on recommended care and screenings. This position may work in a Designated Healthcare Facility which is subject to the State of Maine’s immunization requirements for healthcare workers. If hired, you will be required to show a Certificate of Immunization, or Proof of Immunity, against: 1.) Rubeola (measles); Mumps; Rubella (German measles); Varicella; Hepatitis B; and Influenza.
Schedule: Full-Time, Monday-Friday, 8am-4:30pm (NOTE: Requires in-person presence on a daily basis)
Salary: The salary will be determined based on the credentials and experience of the final candidate. If the finalist is a CNA, MA, or LPN, the salary ranges from $19.50-$27/hour depending on relevant skills, qualifications, and experience. If the finalist is an RN, the salary ranges from $29.50-$42/hour depending on relevant skills, qualifications, and experience.
Highlights of the position:
- Demonstrates compassion, collaboration, communication, and appropriate clinical judgment in patient care.
- Responsible for conducting outreach, scheduling, and verification activities for preventative services and Medicare Annual Wellness visits.
- Performs Medicare Wellness visits with identified patients in person or via telehealth and collaborates with providers for documentation, review, and sign off of completion.
- Is educated on chronic disease management and preventative care, providing related teaching to patients as needed.
- Conducts patient chart reviews for pre-visit planning, medication refills, and other clinical requests. Supports clinical desktop work.
- Is flexible in adapting to the needs of the department, stepping in to perform clinical duties and/or procedures within scope of licensure.
- Creates care plans and strategizes within care team to effectively manage patient’s care.
- Assists in maintaining compliance with quality assurance standards.
- Engages and supports patients to ensure they feel heard, respected, and involved in their care.
- Champions and assists departments with new and ongoing initiatives. Collaborates with all teams to improve the patient experience and clinic processes.

Education and Experience:
- High School Diploma (or equivalent) required. Relevant postsecondary education preferred.
- For CNA or MA - Minimum of two years of experience as a Certified Nursing Assistant or Medical Assistant in a primary healthcare or related setting required. AAMA Certified Medical Assistant certification preferred for MA
- For LPN - Minimum of two years of LPN experience in a primary healthcare or related and valid Maine State Board of Nursing LPN License required for LPN required.
- For RN - One to two years of RN experience in a primary healthcare or related setting and valid Maine State Board of Nursing License as a Registered Nurse required.
- Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
