As the LPN Care Manager, you will play an essential role in providing superior home care services to clients. As an LPN Care Manager, you will work with clients and their families to create comprehensive care plans that address their needs. You will work to ensure that clients have access to the resources necessary to remain healthy and safe at home. In addition to managing clients, you will oversee a team of caregivers, providing mentorship and support to ensure they provide exceptional care.

Responsibilities
  • Create comprehensive care plans that address clients' needs and preferences
  • Oversee a team of caregivers, assigning tasks as needed, and providing mentorship and support
  • Monitor client progress and adjust care plans as necessary
  • Communicate with clients, their families, and healthcare professionals to ensure everyone is informed of the client's condition
  • Maintain accurate and up-to-date records on clients and caregivers
  • Participate in the on-call rotation, providing after-hours support to clients and caregivers as needed
  • Comply with all policies and procedures, including safety protocols

Requirements

  • Active LPN license
  • At least 2 years of experience in home care or another healthcare setting
  • Strong communication and interpersonal skills
  • Ability to assess and analyze situations quickly and efficiently
  • Knowledge of Medicaid and Medicare regulations
  • Ability to work independently and as part of a team
  • Availability to participate in on-call rotation
