Hale Makua Health Services is a Maui based non-profit that improves the well-being of those in its care through compassionate personalized health services. We are devoted to helping Maui's Elders live their best lives. Hale Makua's Kahului location is a 254-bed nursing facility.


The Position:

We're looking for a passionate Resident Assessment Coordinator who will complete and coordinate all aspects of the resident assessment and care planning process.

  • Works in collaboration with the IDT to perform a comprehensive assessment based on standards of nursing practice, federal and state regulations related to the RAI.
  • Oversees appropriate and accurate recordation of information on the Minimum Data Set.
  • Works with the Neighborhood Supervisor and/or Charge Nurse to assure the plan of care is disseminated to all nursing staff.
  • Educates/coaches other IDT members on the technical aspects of the RAI and the care planning process.

Requirements:

  • Graduate of an accredited school of nursing.
  • Two (2) years as a Registered Nurse
  • Experience in long term care preferred
  • Registered Nurse with the State of Hawaii Board of Nursing.
  • Current and valid 2 Step TB clearance and health review (can be done in-house).
