GCHS is looking for a Behavioral Health Therapist who is motivated to provide assessment and diagnosis of behavioral health and chronic care patients. Can create and follow through with measurable treatment plans for patients and identify patients who would benefit from behavioral interventions. This individual is passionate about educating patients, families, and staff on prevention and treatment of chronic conditions and behavioral health conditions. And will maintain patient clinical records in designated EMR in a timely manner. It is preferred that this individual has prior experience working with large and diverse teams to accomplish goals, at least two years of experience in behavioral health services and is bilingual (not required).

Salary
range based on license and years of experience, $58,000-80,000.
