FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.
This is a direct hire, permanent, full-time position in Garrison, NY
Schedule: Monday-Friday, 9am-5pm
The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in a residential facility.
Responsibilities
- Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs
- Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.
- Provides guidance and training to direct care staff to ensure proper delivery of health care services.
- Develops plan of nursing services and oversees the implementation of the health care plan by educating person’s receiving services, DSP/Caregivers and families in the plan of nursing services.
- Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.
- Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
- Provides on call triage coverage via cell phone between the hours of 6:00 am – 6:00 pm, Monday thru Friday. Is prepared to go onsite if an on call problem warrants a nurse’s visit.
- Completes relevant medical documents, assists in developing policies, procedures and forms; and establishing working systems.
Requirements
- NYS RN License, BSN preferred.
- Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
- Excellent oral and written communication skills.
- Able to work flexible hours and/or days.
