Orientation: 04/22 - 23/2024 - ( Month of May Orientation Pending )

Pay Rate:

No housing - Local: $65/hr

With housing (50+ miles away from permanent): $64/hr

Housing stipend: $48/hr (taxable) + $1080 (tax-free) $3000/40hrs (48/27) Over 40hrs=$65/hr base-$97.50 OT
(Must be 50+miles from facility to taxable address to qualify for housing or housing stipend pay)

JOB DESCRIPTION

Provides nursing care and treatment services in South Mountain Restoration Center, a Commonwealth operated nursing facility. The Registered Nurse directs and coordinates the delivery of nursing care and treatment for individuals in resident living area(s). Registered nurses make assessments and decisions regarding staff utilization and the needs of supplies and equipment. Supervises paraprofessional and/or non-professional nursing direct care personnel assigned to their resident living area(s); and ensures that the coordination of services, coverage of services, and maintenance of records is accomplished. Develops, implements, and evaluates individualized care plans. Directs, evaluates, and coordinates the delivery of nursing care and treatment with members of the nursing staff and interdisciplinary teams. Meets with family and residents to assess residents' conditions and completes documentation as required by facility policy, and federal and state regulations.

Responsibility:

• Assesses individuals’ physical, psychosocial and/or mental health status.
• Determines immediate and long-range nursing care and treatment plans, goals, staff utilization, supply and equipment need.
• Assigns and directs the work efforts of assigned paraprofessional and non-professional nursing direct care personnel.
• Coordinates services, coverage of services and maintenance of records.
• Administers prescribe medications and treatments and take physician's orders.

Requirements:

· Resume – with a minimum of 6 months of long-term care experience

· RN PA license (in good standing)

· Current - BLS/CPR

· 2- current references (supervisors)

· 2- step PPD/TB QuantiFERON (within the last 12 months)

· COVID screening and testing consent form

· COVID Vaccination documentation

· COVID Declination

· Fingerprint Letters - Required

· IT Resource user agreement form

· Current physical (within the last 12 months

· MMR, Varicella, Hep B- Vaccination or declined

· PA- Police – criminal background check

· Must be able to pass background/urine drug screen (Within the last 6 Months)
