Communicate with PCP, Inclusa, staff, and family (POA)
Change of condition.
Review Resident's vitals and report abnormal readings to PCP.
Skin assessments upon admission.
Fall assessments upon admission.
Apply nursing procedures as needed.
Work on floor as needed.
Implement interventions of Fall Report.
Complete incident reports.
