WHAT DOES YOUR DAY ENTAIL?

As a vital team member, you lead comprehensive care management, crafting personalized plans. Your duties involve coordinating treatment, breaking down barriers to medical and behavioral health appointments, and proactively engaging with providers. You're a community link, connecting patients to crucial social support, like food, shelter, employment, and legal aid.

You also offer in-person assistance to patients and families, aiding with adherence, providing caregiver guidance, and identifying resources. In crises, you ensure seamless transitional care and assist in discharge planning. Your commitment to health promotion involves educating on independent living skills and empowering individuals to achieve their goals. It's a rewarding, impactful role!
