A Resident Care Coordinator (LPN)

for our newest location, The Inn at Old Saybrook in

Columbiana, Ohio

Responsibilities:

  1. Participate in the developing, maintaining and updating our written policies and procedures that govern the day-to-day functions of the Nursing Services Department.
  2. Direct the day-to-day functions of the nursing staff and manage staffing levels on each unit based on activity.
  3. Ensure that nursing services manual is followed by nursing service personnel, and that it reflects_ the day-to-day nursing care procedures used by this facility.
  4. Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the resident's total regimen of care.
  5. Perform administrative duties such as completing medical forms, reports, studies, charting, etc., as necessary.
  6. Participate in survey inspection made by authorized government agencies.
  7. Admit, transfer, and discharge patients, as required.
  8. Complete accident/incident reports, as necessary.
  9. Respond to complaints and adjust grievances.
  10. Administer disciplinary policy fairly.
  11. Develop and maintain a good working rapport with all interdepartmental personnel to assure that nursing services and activities can be maintained to meet the needs of the residents.
  12. Assure that the wing/unit personnel, residents, and visitors follow the established policies and procedures at all times, including dress code.

Nursing Care Functions:

  1. Maintain an optimum level of nursing care to meet the needs of the patients on assigned wing/unit.
  2. Feed Residents, observe meals served and assure that patients receive diets as ordered by the physicians.
  3. Supervise meal periods in the dining areas.
  4. Answer all call-lights.
  5. Complete orientation of new employees including follow trainings.
  6. In emergency situation; notify physician, DON, Administrator, and patient's family. Arrange transportation and transfer papers for transfer of patients.
  7. Review nurse's notes to ensure that they are informative and descriptive of the nursing care being provided and that they reflect the patient's response to the care.
  8. Follow established standards of nursing practices and implement facility policies and procedures.
  9. Receive and provide report.
  10. Interpret existing policies and procedure to nursing assistants, restorative nursing assistants, patients, families, and physicians.
  11. Safety hazards and initiate corrective action immediately.
  12. Identify patient problems and emergency situation, and initiate immediate "lifesaving" measure, as appropriate.
  13. Follow, supervisor, and evaluate resident's rights.
  14. Report and document changes in patients' conditions to a physician.
  15. Assist with new admissions when necessary.
  16. Audit new admission charts.
  17. Follow admission procedure and communicate patient's admission to other departments, physicians, and ancillary service providers.
  18. Knowledgeably and competently perform basic nursing skills.
  19. Follow discharge/transfer procedures.
  20. Order medications, as necessary.
  21. Observe and enforce infection control procedures.
  22. Ensure personal alarms and safety equipment is in place, turned on, and in working condition.
  23. Participate in patient care.
  24. Other duties as instructed.

Staff Development:

  1. Attend and participated in scheduled training and educational classes.
  2. Attend and participate in scheduled orientation programs and activities.
