Overview:


The role of a Physical Therapist in a Long-Term Care (LTC) facility is crucial in providing specialized rehabilitation services to residents, focusing on improving their physical well-being and functional abilities. The Physical Therapist plays an integral part in creating individualized treatment plans and working collaboratively with the healthcare team to ensure the highest quality of care and support for the residents.
Key Responsibilities:

  • Lead in-house therapy program
  • Create and implement programs for diverse patient conditions
  • Maintain 5 Star Quality Measures

  • Conduct comprehensive patient assessments to identify physical limitations and functional abilities
  • Develop and implement personalized treatment plans to improve mobility, strength, and coordination
  • Provide hands-on therapy, exercises, and activities to enhance residents' motor skills and physical function
  • Educate residents and their families on therapeutic exercises and techniques for at-home care
  • Collaborate with other healthcare professionals to optimize patient care and outcomes
  • Document and maintain accurate patient records and progress reports
  • Conduct regular reassessments of residents' progress and adjust treatment plans as needed
  • Participate in interdisciplinary team meetings to discuss resident progress and care plans
  • Stay updated on latest developments and techniques in physical therapy and rehabilitation
  • Adhere to professional and ethical standards in all interactions and interventions
Required Qualifications:
  • Master's degree or Doctorate in Physical Therapy
  • Valid state license to practice as a Physical Therapist
  • Previous experience in Long-Term Care or geriatric rehabilitation settings
  • Demonstrated knowledge of musculoskeletal and neurological conditions
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and as part of a multidisciplinary team
  • Empathy, compassion, and patience in working with elderly residents
  • Strong organizational and time management skills
