LMSW/LSW Hospice Care PRN

Salary: Starting at $70 Per Visit

Constellation Health Services is looking for a compassionate and empathetic Licensed Master Social/ Licensed Social Worker LMSW or LSW to work with our hospice patients.  This role will involve providing hospice care in the patient's homes and at Skilled Nursing Facilities.

Hospice care or community field care is a MUST!

Per Diem Licensed Master Social Worker (LMSW) or Licensed Social Worker (LSW) in the Rockland, ME! 

$70 Per Visit Weekdays

$80 Per Visit Weekends 

 This Is You:

• LMSW/LSW in the state of Maine with at least 1 year of clinical experience; Hospice, Acute Care, and/or Bereavement Counseling preferred
• Have reliable transportation and be willing to commute within a designated territory to provide the best in-home patient care.

Together, we:

• Provide Clinical Social Work to our hospice population. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plans intervention based on evaluation findings
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system. Submits all clinical documentation, time records and other information in the required time frame
