The Lourie Center for Children's Social and Emotional Wellness seeks to hire a Nurse Specialist who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

The Lourie Center for Children's Social & Emotional Wellness is a private, non-profit agency with a mission to improve the social and emotional health of young children and their families through prevention, early intervention, education, research and training. This position is in partnership with the Montgomery County Infant and Toddler Program.

As a Nurse Specialist you will:
• Work with children ages birth to four and their families as part of the Montgomery County Infants & Toddlers Program (MCITP). The NS provides family counseling and training to families of children that are medically fragile or have a medical diagnosis utilizing a home-based, family coaching model.
• Provide support services or consultation to families and staff regarding child's diagnosis and health issues, including sleep, nutrition, safety, behavior, and other developmental and parenting concerns.
• Provide community resources (healthcare, Medicaid, REM, SSI, DDA, respite) and other supports.
• Educate families about MCITP services and obtain background information as needed for assessment report.
• Participate in eligibility and family assessments and assist in the development of the Individualized Family Service Plan (IFSP).
• Provide service coordination to families and collaborate with other professionals on multidisciplinary teams.
• Attend IEP (Individualized Education Plan) meetings for medically complex children.
Facilitate information sharing and coordination of care between MCITP team and private medical and developmental providers for the child. Request and review medical records for coordination of care.
• Collaborate with outside agencies, including DHHS, MCPS, parent support groups, financial supports, WIC, etc.

Qualifications include:
• Bachelor's degree in nursing required.
• Certification and/or Licensure Maryland R.N. license required.
• Pediatric nursing experience and experience in early intervention and home health care preferred.
• Bilingual candidate preferred.
• Active American Heart Association Basic Life Support (BLS) certification required

Work Schedule:

Monday - Friday 8am-5pm, 20 hours per week
