Overview Full-time Admission Nurse for Winter Haven Hospitals, Weekdays 10 am until 7 pm. Responsibilities Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs. Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team. Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician. Functions as a liaison between disciplines and facilities during the admission process. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate. Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned. Qualifications Current RN license, specific to the state(s) you are assigned to work. Minimum one year of clinical RN experience. Current CPR certification.
