Lead Case Manager

 

Express Employment is seeking a dedicated Lead Case Manager to oversee case management operations within a shelter. This role involves supervising case managers, managing client services, and ensuring the effective delivery of housing assistance to homeless individuals.

 

Schedule: Monday-Friday, 8:30am - 5:00pm
Location: Sacramento
Pay: $21/hour

 

Responsibilities:

  • Supervise and support Case Managers within the Emergency Shelter.
  • Assign daily tasks and oversee case management activities.
  • Manage provision of case management and housing services to shelter clients.
  • Maintain accurate logs, reports, and client files.
  • Coordinate client appointments and ensure quality service delivery.
  • Input and generate HMIS reports for shelter contracts.
  • Develop and curate community referral resources.
  • Coordinate with external agencies to ensure client readiness.
  • Establish and manage housing referral system.
  • Conduct client workshops on housing-related topics.
  • Maintain collaborative relationships with stakeholders.
  • Track and review client data for program effectiveness.
  • Submit reports and paperwork as per supervisor''s instructions.

Requirements:

  • High School Diploma or GED equivalent.
  • AA Degree or minimum five years related experience.
  • Strong organizational skills with attention to detail.
  • Excellent written and verbal communication skills.
  • Proficiency in Word and Excel; HMIS experience desirable.
  • Valid driver''s license and clean driving record.
