New Beginning HCSP is a Medicaid Waiver Program Provider based in Harlingen, Texas that supplies services and support to Texans with Intellectual and Developmental Disabilities. Our doors have been open to the community since May 2019 and we hope to continue serving our fellow Texans for years to come.

Job Summary
We are seeking a Case Manager to join our team. In this role, you will work collaboratively with Clients, Families, Service Coordinators and onsite Direct Care Staff to determine a client's needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about Home and Community-based (HCS) practices.

Responsibilities
  • Coordinate a variety of program services including Host Home Care, Supervised Living, Speech Therapy, Behavioral Support, etc
  • Develop individualized care plans
  • Educate clients and families on their rights and treatment plan options
  • Create goals and monitor progress toward goals
  • Write and produce professional quality internal and external communication such as emails, paper forms, and plans.
  • Manage client services in the form of emails, meetings, phone calls, communication records, etc.
  • Coordinate with Tropical Texas service coordinators and other State of Texas level agencies
  • Maintain compliance with Texas HCS guidelines (soon adding TxHML)
  • Meet deadlines
  • Perform home walk-throughs and inspections
  • Become an integral point of contact for clients, guardians, and families
  • Enter forms and manage approvals through TMHP (training will be provided)
  • Lead and facilitate client and guardian meetings when needed
  • Advocate for your clients and their needs
Qualifications
  • Previous experience as a Case Manager or in a similar position (ICF, CLASS, DAHS, etc) is preferred
  • Strong problem-solving, time management, and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
  • Feel comfortable dealing with a minimum of 20 clients and their families/guardians
  • Ability to navigate through Outlook, Word, Excel. (Sharepoint, TMHP, Teams preferred)
  • Ability to communicate with families and guardians in both English and Spanish
