San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

This is a full-time position in our PACE Clinic in Chula Vista, CA.

Job Summary:
Under the supervision of the Center Director, manages flow of clinic leading all staff in provision of medical and nursing care, ensuring high quality standards. Coordinates services with primary care team, acts as primary liaison. Triages, and assess participants as needed. Acts as gatekeeper for daily clinic scheduling. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family, and staff teaching and communicates participant’s condition to caregivers, primary care, and the interdisciplinary team. To ensure continuity of operations assumes the Center Directors responsibilities in her/his absence.

Qualifications & Requirements:
• Education - Graduate of a school of professional nursing program required. BSRN strongly preferred. Current California Registered Nurses License and CPR certification required. Shall be at least 18 years of age.
• Experience – A minimum of 1 years experience working with a frail or elderly population required. Two years of nurse management experience in a hospital, nursing home or community-based setting preferred. Shall have either training or related experience in the job assigned.
Duties and Responsibilities:
• As needed, conducts an in person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
• As needed conducts an in person nursing reassessment semiannually and as needed.
• Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery.
• Provides orientation, initial and annual competencies, supervises and evaluates performance of Clinic R.N.’s, L.V.N.’s, C.N.A’s, and Clinic Clerk. Has indirect supervision of personal care provided by Day Center Workers
• Performs competencies on appropriate Clinic staff as needed.
• Ensures that the job responsibilities, authorities, and accountability of direct reports are defined and understood. Ensures that clinic staff works with in their scope of practice.
• Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinic staff to function independently.
• Evaluates the overall effectiveness of the clinic, implementing change and quality improvement as needed.
