Job Summary:

The Field Supervisor at Community Connection Programs, Inc. plays a crucial role in ensuring the delivery of high-quality home care services to our clients. This position involves overseeing a team of caregivers, monitoring client care plans, and maintaining compliance with regulations and agency policies. The Field Supervisor will work closely with the Regional Program Director to ensure efficient operations and exceptional client care.


Key Responsibilities:

  • Supervision and Team Management:
  • Supervise a team of caregivers, including scheduling, training, and performance evaluations.
  • Provide guidance, support, and coaching to caregivers to ensure they meet the agency's standards for client care.

Client Care Oversight:

  • Conduct regular home visits to assess client needs and ensure care plans are being followed.
  • Collaborate with caregivers to develop and update individualized care plans.
  • Address client concerns and inquiries promptly and professionally.

Compliance and Documentation:

  • Ensure compliance with all state and federal regulations, as well as agency policies and procedures.
  • Maintain accurate and up-to-date client records, including care plans, progress notes, and incident reports.
  • Conduct regular audits of caregiver documentation to ensure accuracy and completeness.

Quality Assurance:

  • Monitor the quality of care provided and identify opportunities for improvement.
  • Participate in quality assurance activities, including client satisfaction surveys and performance reviews.

Emergency Response:

  • Act as the point of contact for caregivers and clients in case of emergencies or urgent situations.
  • Ensure that caregivers are trained in emergency procedures and response protocols.

Communication and Coordination:

  • Maintain open and effective communication with the Home Care Agency Manager, caregivers, and clients.
  • Coordinate with the scheduling department to ensure appropriate caregiver assignments.

Training and Development:

  • Assist in the orientation and training of new caregivers.
  • Provide ongoing education and support to caregivers to enhance their skills and knowledge.

Qualifications:

  • Bachelor's degree in healthcare, social work, or a related field preferred.
  • Previous experience in home care, healthcare, or supervisory roles is highly desirable.
  • Knowledge of home care regulations and best practices.
  • Strong interpersonal and communication skills.
  • Excellent problem-solving and decision-making abilities.
  • Compassionate and dedicated to providing quality care to clients.
  • Ability to work independently and as part of a team.
  • Reliable transportation and a valid driver's license.

Working Conditions:

  • This role involves a combination of office work and field visits to client homes.
  • Must be available for on-call responsibilities and occasional evening or weekend work.
  • Physical requirements may include standing, walking, and lifting up to 25 pounds.
