Health Home Case Manager Job Description:

We are seeking a dedicated and compassionate individual to join our team as a Health Home Case Manager. As a Health Home Case Manager, you will be responsible for coordinating care for individuals enrolled in our health home program who have complex medical and behavioral health needs.

Key Responsibilities:

- Conduct comprehensive assessments to identify the unique needs and goals of each client
- Develop individualized care plans in collaboration with clients, healthcare providers, and other members of the care team
- Monitor and track progress towards achieving client goals and outcomes
- Coordinate and facilitate access to medical, behavioral health, and social services
- Provide education and support to clients and their families on managing chronic conditions and navigating the healthcare system
- Advocate for clients to ensure they receive high-quality, coordinated care
- Maintain accurate and up-to-date documentation of client interactions and progress in electronic health record systems
- Participate in team meetings and case conferences to discuss client cases and develop care plans

Qualifications:
Master's degree in social work, nursing, psychology, or a related field
- Active licensure as a social worker, nurse, or other healthcare professional
- Previous experience working with individuals with complex medical and behavioral health needs
- Strong communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Proficient in Microsoft Office and electronic health record systems
- Bilingual or multilingual skills are a plus
