Job Description
Scope
Under the direction and supervision of the Home Care Manager, the Personal Care Worker (PCW) is an initial contractual position that is responsible for providing and assisting clients with activities of daily living and supportive care necessary to the preservation of the home environment. The PCW will assist in monitoring the health status, providing a safe and positive living environment to enhance the client’s quality of life.
Job Responsibilities
· Provide unskilled personal care services and homemaking services.
· Accompany clients to appointments, on errands and shopping as appropriate.
· Provide assistance or stand by assistance with bathing, skin care, hair care, toileting, dressing, shaving, nail care, mouth care, etc.
· Assists with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care)
· Observe clients taking prescribed medications.
· Clean stove, kitchen, bathroom, and only the client’s living area and bedroom
· Vacuum, sweep, dust, take out trash, run errands as needed, mop floors.
· Advises Program Manager of requests which cannot be met or of conditions which are unsafe.
· Record and report observed noticeable changes in client’s physical condition, behavior, or appearance immediately to Program Manager and/or nurse.
· Documents services provided to clients consistently and accurately. Documentation is completed in a timely manner.
· Participates in agency required and assigned training programs and any required staff and training meetings.
· Document daily activities and report any significant changes in a client’s needs or living conditions.
· Additional responsibilities may be added as appropriate.
Requirements:Essential Job Requirements
- Minimum High School Diploma or GED
- Ability to lift, push, or pull 25 pounds.
- Ability to bend, twist, stoop, kneel and reach.
- Ability to read, analyze and interpret regulations and other documents.
- Must have reliable transportation.
- Possess a valid state issued driver’s license and good driving record.
- Possess valid auto insurance.
- Ability to treat and care for clients and their property with dignity and respect.
- Ability to adapt to various living environments and locations.
- Ability to communicate with clients in a friendly and congenial manner.
- Ability to communicate with and listen to clients for accurate needs assessment.
- Ability to read and follow written instructions and document care given for the Plan of Care.
- Self-starter with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail, and the impaired.
- Maturity and ability to deal effectively with the demands of the job.
- Demonstrate professional skills.
Experience:
· Preferred Home Care experience in an agency setting, facility, or with family.
· If no experience, then CNA license is needed.
License:
