Springwell is adding a new program! Come be a Liaison to help individuals of all ages transition from Long-Term Care facilities to community living situations!

POSITION DESCRIPTION:

A Community Transitions Program Liaison:

  • Visits with skilled nursing facility residents to introduce program and increase awareness of services
  • Identifies individuals appropriate for the program based upon a resident’s desire to transition to a community living environment
  • Facilitates person-centered planning and needs assessments through interdisciplinary team meetings
  • Acts as an onsite point of contact for residents, families, skilled nursing facility staff, and all other parties involved with a resident’s care for transitions to the community
  • Completes referrals to programs and follows-up on referrals to ensure an effective community discharge  
  • Coordinates with other state programs and teams who will support the individual’s transition to the community, including screening for various community waiver programs

QUALIFICATIONS:

  • Bachelor's Degree or higher.
  • Applicants without a Bachelor's Degree may be considered if:
    • The applicant has an Associate's Degree and at least 3 years' experience working with elders or providing case management services; OR
    • The applicant has completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3 year period.   
  • Knowledge of long-term care, case management, discharge planning, community resources, programs, and benefits to help support an individual’s transition from a community setting
  • Customer service skills and experience working with a diverse population
  • Applicant must have a reliable car, current driver's license, and safe driving record.
  • Proficiency with computer programs including Microsoft Word, Excel, and Outlook
