Nurse Practitioner - Health Risk Assessments - PRN

We are looking for Nurse Practitioners who are interested in a flexible side job. This per diem job allows you to customize your schedule and earn supplemental income while maintaining an autonomous relationship.

**PRIMARILY LOOKING FOR PROVIDERS WILLING TO SEE PATIENTS IN HILLSDALE COUNTY**

Responsibilities of the Nurse Practitioner - Health Risk Assessments:

• Conduct comprehensive in-home or remote health risk assessments on Medicare Advantage members
• Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
• Educate and counsel patients on identified conditions and screening procedures
• Document findings using the provided iPad
• No continuum of care, prescribing, on-call, or follow-up is required

Requirements of the Nurse Practitioner - Health Risk Assessments:

• Active and unencumbered Nurse Practitioner license
• Minimum availability of 12 hours per week (including one full 8 hour day) is required to qualify
