One (1) full-time (40 hours/week) Case Manager opportunity working with Merced's Road to Resilience program in Merced, CA. The Case Manager/Navigator will work with pregnant women who known histories of substance use, pregnant women with current substance use, and mothers of substance exposed infants through interagency collaboration and integration of services. The overarching goals is to take steps to eliminate substance use and prevent relapse and increase child outcomes associated with parental substance use through in home support and care.

Qualifications:

  1. Bachelor's degree in counseling, social work or related field preferred.  May accept equivalent experience.
  2. Knowledge of community resources.
  3. Cultural sensitivity, awareness and knowledge.
  4. Sound personal qualifications (i.e., maturity, stability, high moral standards and sound mental health).
  5. Strong Communication Skills
  6. Clean and neat appearance and pleasant demeanor
  7. Must be 21 years old or over.
  8. Must possess a valid California driver’s license with less than 2 minor moving violations.
  9. Bilingual in Spanish preferred.

Responsibilities:

  1. Outreach to the community about the R2R program and provide linkages and coordination of services.
  2. Follow up phone calls and home visits including collaboration with referring agents or previous providers.
  3. Conducts and participates in R2R specific community meetings.
  4. Plans, develops and evaluates activities for R2R clients.
  5. Utilize a screening tool to assess the level of risk and refer to appropriate resources.
  6. Administer program measurements tools and surveys to track and report program data and outcomes.
  7. Provide documentation on each contact with child/family.
  8. Attend staff meetings and trainings as requested.
  9. Report any suspected child abuse immediately to your direct supervisor or if not available up the chain of command.  
  10. Report any suspected adult/elder abuse immediately to your direct supervisor or if not available up the chain of command.
