GENERAL RESPONSIBILITIES: The Qualified Nutritionist at Whitney Young Health develops and implements the Nutrition Services and Education component of the Special Supplemental Food Program for Women, Infants and Children (WIC). This role provides nutrition education and counseling to participants of the program. SPECIFIC RESPONSIBILITIES: Acts in the capacity of the Qualified Nutritionist (QN) by providing participant centered nutrition services and high-risk counseling to WIC participants. Performs all other QN duties as necessary. Design and implement an educational program that is culturally sensitive and cognizant of the unique needs of the clients. Assists in the development of nutrition education materials for individual and group counseling using NYS-DOH resources for guidance., such as the NYS Healthy Lifestyle Toolkit, “Talk, Listen and Connect”. Acts as a high -risk nutritionist and implements, maintains and completes high risk care plans for high-risk participants. Assigns nutritional high-risk criteria to high-risk participants. Updates High Risk Care Plans at all subsequent appointments. Contacts medical provider when formula medical documentation forms are not filled out correctly or the condition does not match the formula prescribed. Acts as a liaison between the participants and medical providers. Assigns and tailors food packages as necessary and loads benefits on participant’s eWIC card. Issues eWIC cards to participants and helps them set up their pin. Explains the WIC2GO application and helps participants set it up on their phone. Perform all clerical data entry in WICSIS NYWIC including scheduling, category eligibility, residency eligibility, income eligibility, answering phones and determining nutritional risk eligibility. Perform anthropometric and hematological measures for WIC participants as required by DOH standards and policies. Generates prints and scans all mandatory documents in participant’s electronic record. May act as the breastfeeding Coordinator in the Breastfeeding Coordinators absence. Supports the Breastfeeding Peer Counselor program and promotes breastfeeding activities. Provides education, support and referrals to moms experiencing breastfeeding complications. Maintain up to date knowledge of programs and services available at Whitney Young Health Center as well organizations in the community and makes referrals to appropriate agencies when indicated. Documents and follows up on all referrals in NYWIC per DOH standards and policies. Recruits WIC participants to become Whitney Young Health patients and schedules medical and dental appointments as necessary. Attend and participates in mandatory and other appropriate trainings as directed by the WIC Program Manager. Develops and follows yearly training plan. Records and submits vendor and customer complaints to the WIC Program Manager. Travel to WIC satellite sites and loads and unloads equipment. Participates in outreach activities. Demonstrates excellence in both internal and external customer service. Share in daily maintaining of a professional appearance of the WIC waiting area and keeping children’s toys clean with the use of approved disinfectant wipes. Performs daily maintenance of HemoCue equipment, maintains log of daily maintenance. Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality. Work late clinic hours as directed by WIC Program Manager. Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH). Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center. Completes other duties as assigned. Requirements MINIMUM QUALIFICATIONS: Bachelor’s Degree (BS) in food and nutrition. Must meet all of the qualifications of a Qualified Nutritionist as outlined by the State of New York. Must have a valid NYS driver’s license. Flexibility to work evenings and weekends, as needed. PREFERRED QUALIFICATIONS: Master’s Degree in nutrition. Registered dietician and certified lactation counselor (CLC).
