Description:

We are a growing, fast-paced physician-led office that is a leader in our field of treating kidney disease looking for a full-time Advanced Practice Provider. Our clinics are open Monday through Friday with no weekend or holiday requirements. The right candidate will thrive in a multi-tasking environment serving our patients and serve as a partner with our team of excellent physicians and APPs. A few of the key duties of this role will include comprehensive management of patient conditions including assessment, diagnosis, treatment, and consultation, on a longitudinal basis of chronic kidney disease patients. The provider will help educate and engage patients in their disease process, with the ultimate goals in mind of prolonging disease progression, decreasing hospitalization/ED utilization, assisting with optimal starting of dialysis, and engaging patients in Home Dialysis therapies. Our providers conduct Comprehensive Disease Assessments (CDA) to capture the patient's whole health story and identify gaps in care. We are partnered with Evergreen Nephrology and their Interdisciplinary teams to identify and eliminate social determinants of healthcare for our patients, including functional capacity, home environment, health literacy, housing instability, food, insecurity, and caregiver support. About 70% of this role will spent in our CKD offices (including some travel to satellite offices) and 30% in a dialysis unit.

Requirements:

Completion of an accredited program for Nurse Practitioner (NP). Two+ years of experience working in a value-based care (VBC) model, including documenting/coding from an MA risk perspective and conducting Health Risk Assessments (HRA) on chronically ill patients.
