Intake Assessor (LSW, LISW, LPC, LPCC) - Full-time

As an Intake Assessor, your pivotal responsibilities include conducting psychosocial assessments, determining suitable levels of care, and facilitating referrals to specialized programs that address the distinct needs of clients. You will be in charge of overseeing the intake process for individuals seeking services at The Crossroads Center, ensuring precise and prompt documentation.

Key Responsibilities:

• Conduct psychosocial assessments for individuals seeking services at The Crossroads Center.
• Demonstrate proficiency in the Diagnostic and Statistical Manual of Mental Disorders (Current Edition) and American Society of Addiction Medicine Criteria to make well-informed decisions regarding the most suitable level of care for service requests.
• Actively participate in team meetings, engage in ongoing education, and training opportunities to enhance professional expertise.
• Collaborate with clinical staff to address any clinical challenges that may arise during the intake process.
• Maintain accurate documentation in compliance with agency standards.
• Attend required team and other meetings.
• Provide exceptional customer service to both internal and external stakeholders, resolving service issues within available resources and escalating complex matters when necessary.
• Perform additional assigned duties as required.

Experience & Qualifications:

• Minimum of a bachelor’s degree in social work, counseling, or a related field; Master’s degree preferred.
• At least 2 years of experience in intake and/or conducting assessments for adults and adolescents with substance use disorders and/or mental health disorders.
• Certificates, Licenses, Registrations: Licensed in the State of Ohio as an LSW, LISW, LPC, LPCC.

Physical Requirements:

While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. (The employee must occasionally lift and/or move up to 15 pounds). Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
