The Opportunity: EMRGE Re-Entry Case Manager

Job Type: Full-time, On-Site

Pay: $50,000- $52,000k/ year

As Case Manager, you'll have a dynamic role working with clients currently incarcerated and being released from the Essex County Correctional facilities.The Essex Medication Re-Entry Grant Expansion Program (EMRGE) is a program for incarcerated and/or justice-involved individuals with opiate use disorder. The program helps participants overcome barriers such as employment assistance, recovery, community support and mental health services. As Case Manager you will help with the client's transition back to their community, and follow-up with their goals and action plans 6 months post release. As Case Manager, you will work closely with the Program Manager and a dynamic team of clinicians, and correctional staff to ensure that clients receive the care and support they need to be successful.


This role is about making a difference in the lives of the people we serve. You'll bring the following qualifications:

  • BA Degree in psychology, social work, counseling, or related human service field preferred; however, a minimum of two years of relevant experience working with justice involved population, behavioral health and substance abuse disorders, or equivalent can be substituted.
  • Two years experience working with inmate population required with relevant experience with behavioral health and substance abuse disorders. Related practicum or internship placement/work may substitute for the required experience.
  • Previous experience with coordinated care planning in criminal justice preferred.
  • Ability to pass background and clearance process for a Correctional facility
  • Demonstrated excellent written and verbal communication, organization, and computer skills.
  • You must be willing to travel within the community to deliver services on a mobile basis in suburban and urban areas. Staff may be asked to travel to multiple locations in a day and you must have a valid driver's license and a reliable vehicle.

Hours: 8am- 4pm


Key Responsibilities:

  • Carry a caseload of up to 30 inmate clients who have been enrolled in the EMRGE Program and are being released from the Essex County Correctional facility. Work with clients 6 months post release to meet desired goals, addressing and overcoming barriers to services.
  • Participate in post-release transition planning, including aftercare and supportive services, to ensure services address needs across life domains (education, employment, housing, etc.).
  • Conduct further screening and assessments of referred inmates, and assist in diagnostic evaluations. Receive training and exhibit proficiency administering the ORAS, CAAPE-5, and MSDP assessment tools.
  • Provide services and meet with clients in the community, at the VOAMASS offices and VOAMASS residential programs. Some caseloads may require you to attend court, conduct housing searches, and attend other identified services. Telehealth can be used as appropriate; however, this is allowed on a case-by-case basis based on clients' needs and requests.
  • Establish and maintain professional and consistent working relationships with the integrated services care team and coordinate care with other providers across the criminal justice system. Ensure program participants are engaged and supported in services that include Medication-Assisted Treatment (MAT), and group and individual therapy.
  • Attend weekly program evaluation meetings.
  • Maintains records through our electronic record keeping system according to VOAMASS Policies and Procedures. Complete all documentation and authorizations promptly, and comply with professional standards as required.
