Nurse - MH LVN-LPT (985)
Hanford, CA

 

Provides nursing services for clinic.  Oversee patients' health needs and provide direct care and health education as indicated. Assist physician during scheduled medication clinics.  Assessment of patients both behaviorally and physically and intervene when necessary.  Record pertinent data in patient's chart including the following: a. attendance for billing purposes; b. blood pressure; c. weight; d. administer medication prescribed by physician accurately and precisely; e. plan and implement when indicated health and hygiene training with patient and staff; f. include patient family members in translating treatment plans or clarifying patient's health need. Work with pharmacy on refill medications and provide injections to consumers. Will conduct home visits, provide patient education and assist with medications as needed.

 

How will I contribute

  • Communicate with physician and clinic staff and community agencies involved in patient’s treatment/care.
  • Work with supervisor for modification of existing nursing responsibilities so that all responsibilities are fully covered.
  • Provide aggressive follow-up on patients, participation regarding continuation of medications.
  • There may include some evening hours based on program need.
  • Abide by the clinical rules and regulations of the agency.
  • Maintain a productivity standard of 100 billable hours per month.
  • Complete clinical documentation on the same day as service, at a frequency of 80%.
  • Communicate with and translate for Spanish speaking consumers as needed, if position requires it.
  • Contribute creative ideas to make program more effective.
  • Achieve basic competence in using our electronic record keeping system. Includes but is not limited to the following:

*             Effective use of EHR Scheduler

*             Effective creation of Program Notes

*             Effective use of Clinician’s Home Page

  • Achieve all performance standards.
  • Complete 4 hours of cultural humility training per year.
  • Serve on selected committees and boards as directed by supervisor.
  • Maintain active status with the California Board of Nursing as a Licensed Psychiatric Technician.
  • Work collaboratively with local Board and Care facilities in order to address consumer needs so that more expensive placements can be avoided.
  • Maintain familiarity with and adherence to compliance with the code of conduct and the requirement of the compliance program. These are elements of performance evaluations and conditions of employment.
  • In the course of performing job duties, follows all Kings View policies and procedures, administrative directives, all relevant county, state and federal regulations, and any performance obligations contained in Kings View program contracts with other entities.
  • Conduct home visits
  • Provide medication education and linkage to services as needed.
  • Other duties as assigned.

 

What I bring

  • High School Diploma (or Equivalent)
  • Class C Driving License Required
  • Graduate of an accredited LPT. nursing program
  • Valid and current California LPT or RN license
  • 1-3 years of experience
  • Working knowledge of medications especially psychotropic drugs and their administration in a psychiatric setting
  • Ability to do home visits
  • Requires sensitivity in relating to clients, families, staff and community.
  • Skills in effective communication both written and oral.
