Our client is seeking an LPN Wing Supervisor to carry out the medical plan of care and oversee the Nursing Assistants' care of residents. The ideal candidate will have a passion to work with and interact with residents daily. This role involves administering medication, transcribing physician's orders, completing monthly summaries, and supervising and assisting with post mortem procedures. The LPN Wing Supervisor will also be responsible for maintaining a clean, safe, and healthy environment and ensuring that staff is familiar with changes in policies and procedures.

Key Responsibilities
● Administers and records medication, treatments and monitoring e.g. glucose checks, as ordered and reports and documents resident response
● Accurately transcribes physician's orders
● Completes monthly summaries as scheduled
● Reviews the care plan with NAs on monthly summary day and updates the care plan to reflect the resident’s current condition.
● Notifies the RN Supervisor immediately of a significant change in resident’s condition
● Reports and records changes in resident condition to the RN Supervisor
● Communicates resident transfers to immediate family and via email to department heads
● Completes transfer form for all transfers
● Updates care plans with clear, concise, time limited goals to reflect resident condition and directs team to implement approaches
● Assists physicians with rounds, examinations and treatments as needed
● Collects laboratory specimens e.g.: urine, cultures, stool, etc. and arranges for delivery to Hospital with accurate and complete requisitions. Documents in PCC when laboratory work is done
● On resident death, supervises and assists with post mortem procedures. Assists with collection of resident's clothing and belongings. Notifies all department heads of death via email, makes phone calls as directed by RN Supervisor
● Encourages resident participation in scheduled activities
● Reports to team changes in resident condition, care plan changes and any special assignments to be carried out
● Assists RN Supervisor with annual evaluations of nursing assistants
● Along with RN Supervisor, writes commendations and counseling records as needed
● Accurately completes incident reports or assists team members in writing report
● Documents weights and vital signs and follows up as needed, including reweighing residents if there is a three pound or more discrepancy from the last weight
● Cleans and stocks treatment trays daily
● Prepares daily report for the wing
● Counts narcotics with another nurse at beginning and end of shift
● Maintains an accurate inventory of resident belongings - notes new items brought in and items removed from facility
● Maintains work area in clean, orderly condition. Performs housekeeping assignments as scheduled
● Submits maintenance requests as needed
● Maintains a clean, safe and healthy environment and reports potential safety hazards
● Supervises and directs resident care by nursing assistants to insure the prescribed plan of care is carried out
● Performs additional duties as directed by supervisor
● Attends care plan conference for residents on assigned wing if requested
● Assists in orientation of new nursing staff
● Adjusts staffing assignments as needed to insure good resident care
● Conducts team conferences as needed
● Assures that staff is familiar with changes in policies and procedures
● Updates the daily census as needed
● Monitors NA documentation for completeness and compliance with regulations
● Collects data for quality assurance studies as directed
● Monitors nurse aide job performance for compliance with infection control practices
● Provides direct care for residents as needed, within restrictions
● Seeks clarification or guidance to perform job responsibilities when in doubt
● Documents drug disposal on resident charts in PCC.
● Attends in-services, department and staff meetings, committee meetings as required
● Assist CNA’s on the hall as needed

Schedule and Shift Details
● Days 6AM - 6PM // Two days on // Two days off // Three days on // 84-hours biweekly pay period, Rotating Weekends


qualificationsrequired
  • Associate degree or higher
  • Valid American Heart Association Basic Life Support (AHA BLS)
  • Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
