Description: This is an entry-level position in the HIS Department. A person in this classification deals with very confidential materials and errors could be of considerable consequence. Patient records are kept in an organized manner and under tight security.

Primary Responsibilities:
Always protects the confidentiality and access to patient health information.

Demonstrates care and accuracy when handling patient health information.

Daily demonstration of courtesy and consideration with patients, providers and co-workers.

Answers incoming phone calls.

Accurately performs all required procedures on dictation of progress notes.

Accurately performs all required procedures for processing release of information requests.

Performs other duties deemed necessary and assigned by the supervisor.


Qualifications: Excellent communication and organizational skills with ability to pay attention to detail. Experience in clinical record management is desired. Must be flexible and a team player.

Working knowledge of patient record systems.
Working knowledge of health care terminology.
Typing skills of 30+ wpm, minimum

Skill in using manual, mechanical and electronic equipment.

Skill in handwriting and note taking.

Skill in using good interpersonal communication techniques to establish and maintain effective working relationships with peers, patients, providers and the public.

Skill in self-motivation, problem solving and accuracy.

Ability to maintain positive attitude under stressful conditions.

Minimum Requirements:

High School Graduate or GED equivalent


Health care records experience preferred.

Basic office skills including organization and communication.
