**Registered Nurse (RN) - Hospice Case Manager**

Location: Harrison, AR Department: Clinical

Legacy Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Hospice Case Manager. In this role, you will collaborate with patients, families, and healthcare professionals to provide exceptional care during the most crucial stages of patients' lives. **Key Responsibilities:**
- Conduct comprehensive assessments of patients and families to determine hospice care needs.
- Develop, initiate, and revise care plans based on patient needs and status.
- Administer medications and treatments as prescribed by physicians.
- Offer counseling and support to patients and families, including education on self-care techniques.
- Coordinate patient care and communication with interdisciplinary teams.
- Participate in on-call duties and ensure necessary equipment and services are available.

**Key Qualifications:**
- Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and active CPR certification.
- Valid driver's license, personal automobile, and insurance coverage.
- One year of hospice care experience is preferred.
- Case Management experience and supervision of hospice aides is a plus.
- Excellent observation, communication, problem-solving, and nursing skills.
- Ability to perform physical tasks such as lifting, positioning, and transferring patients.
- Effective social interaction, communication, and organizational skills.
